We introduced Intradiscal Ozone Discectomy (Ozonucleolysis in 2004, as an alternative to surgery for the treatment of back, Leg, or Arm Pain caused by Herniated Discs. So far 61,000 patients have been treated successfully using injections of Medical Ozone with a success rate of 80-95% with no complications.

Ozonucleolysis is done without anesthesia and the patient goes home after a short recovery on the same day. The patients may need 1-5 sessions during 8 weeks. The patients are spared from prolonged absence from work and procedure-related disability.

How does Ozone work?

When Ozone is injected into the inner disc space nucleus pulposus, there is a production of antioxidant enzymes. These neutralize the toxic materials present inside the inner disc space and cause shrinkage of the disc. Further, Ozone dissolves in the water in the disc and affects the proteoglycans which are the building blocks of the disc causing them to shrink thus reducing the disc volume and lessening the pressure on the nerves.

Is Ozone therapy safe?

It is safe in hands of interventional neuroradiologists.  Ozone injections have been found to be an extremely safe form of treatment and are currently employed as the first treatment of choice in disc herniation in many countries. The treatment is by and large free from any side effects of the injection of Ozone itself.

Benefits of Ozonucleolysis

Benefits of Oxygen-Ozone Treatment

Besides a shorter recovery time and lower complication rate than surgery, other benefits of oxygen-ozone injections include:

  • It is minimally invasive.
  • It is an outpatient procedure.
  • It takes little time to administer.
  • It costs much less than surgery.
  • It is a pain-free procedure.

Who should consider it?

Patients with:

  • Very severe back, leg, and or arm/shoulder pain due to disc herniation
  • Pain that has not responded to 4 – 6 weeks of conservative treatments(rest, pain medications, physical therapy)
  • Herniated lumbar or cervical disc confirmed by imaging studies (CT scan, MRI, and Myelography)
  • Low back pain where the disc appears desicated on MRI or CT

Who should not consider it?

Patients with:

  • Back pain where the disc appears normal by MRI/CT scan
  • Cancer or other spinal tumors
  • TB or other spinal infections
  • Severe spinal stenosis due to degenerative changes in the spine and the discs
  • Spondylolisthesis (2nd or 3rd degree) causing spinal instability

What is the course after the Ozone treatment?

  • Bed rest advised for the first 48 hrs
  • 10-15 min walk advised from day 10
  • Full activity with the exception of vigorous sports and lifting allowed from day 8
  • Most people return to work by day 8
  • Forward bending and lifting of weight not allowed for 4 weeks
  • Spinal exercises are  to be started only after 3 weeks and after consulting the doctor

Ozonucleolysis in cervical Radiculopathy

Background & Purpose:

Direct injection of oxygen-ozone in the cervical discs has proved to be an effective alternative to surgery
in patients with cervical disc herniation in many countries around the world. We report our experience with ozonucleolysis with
patients affected by pain in the cervical region (Brachalgia) due to disc herniation including post-operative recurrence or disc prolapse.

Methods:

4000 patients were treated with a single session of oxygen-ozone therapy from 2005-2016. All the patients had CT or MRI
evidence of cervical disc prolapse with clinical signs of cervical nerve root compression. The procedure was performed under angio
fluoroscopy using a 22/23 G spinal needle without any form of anesthesia. All the patients received intra-discal injections of an oxygen-ozone mixture at an ozone concentration of 30 µgm/ml. Among 4000 patients, 3000 were males and 1000 were females between the ages
of 20-70 yrs. Therapeutic outcome was assessed 5 months after treatment by using the modified MacNab method.

Results:

A satisfactory therapeutic outcome was obtained. 60% of the patients showed complete recovery with the resolution of symptoms.
20% of the patients complained of occasional episodic neck pain and arms pain with no limitation of occupational activity. 5% of
cases showed insufficient improvement. 5% of cases had no improvement and went for surgery. 10% of cases never turned up after
the first visit.

Conclusion:

Intradiscal and periganglionic injection of ozone for the herniated cervical disc has revolutionized the percutaneous approach
to nerve root diseases making it safer, cheaper, and easier to repeat than treatments currently in use.

Biography

Umair Rashid, the first Pakistani obtained a high qualification in Intervention Neuropathology from the European Board of Neuropathology (ECIINR). He is one of the pioneers in establishing the First Brain Stroke Centre in the Country at Lahore General Hospital/ Post-graduate Medical Institute, Lahore. He has also recently introduced the latest noninvasive,
non-surgical treatment of Backache, Disc Lesions, with Oxygen Ozone Therapy in the Country. He already performed 19000 cases including Cervical & Lumber Spine
Discs.
